I created an image which is 4.25 x 6 to match the custom notecard paper I have (4.25 x 6).

When I go to print, my image is either CLIPPED or reduced to fit media" such that it doesn’t print out properly (not centered and not big enough).

If I manually bump up the size (%), then image gets clipped again.

WHY IS THIS HAPPENING TO ME????

Probably because your printer can’t print all the way to the edge of the paper?

Most injets can’t because of the need for unprintable areas to facilitate paper handling. Some can print borderless on *some* sizes of paper.
