Stroke path looks like a Holstein cow

Drawing a constrained rectangle with the pen tool, when I went and stroked it with the pencil tool at 1 pixel, one small corner @ top right was darker. The majority of the stroke was a light shade of gray. I had the default B&W on. The dark was actually scattered around the perimeter, hence the Holstein reference.

What setting have I screwed up this time ?
