It may be your brush size is too large, as BobS suggests, or you may just need to dump your preferences. A good way to locate your cursor, is to press the space bar, which will convert the cursor to a hand. At least you will be able to see where it is. If you’re using your brush (or eraser) , you may have your Caps Lock on which will change it to cross hairs.
To dump your prefs, in Windows, press and hold Alt+Control+Shift immediately after launching Photoshop or with a Mac OS, hold down Option+Command+Shift.
