Using eyedropper to set level points

1. Is there a general rule of thumb as to when you should adjust levels
using the histogram VS. setting the dark and light points using the eyedropper?

2. Using levels, how do you set the mid point with the histogram? Where should it be dragged to?

3. How do you sample the mid -range color using the eyedropper? The darks and lights are obvious, what about the mid range?
