Is your original any bigger ? The one you attached is not only small … but a low resolution too. The image is very poor to start with. However, given what you gave us to work with … first try using the patch tool on the splotchy areas. Then try using the High Pass filter & then Overlay in blend modes. I’d also suggest using the Lens Blur filter on the background … so that the man appears to be sharper by contrast. Oh wait … you are still using a version of Photoshop from the Windows 95 days. So one final suggestion …upgrade. I am using Photoshop CS and did not have a tremendous amount of success with it . To make a long story short … if I can’t do much with Cs, an old version like 5 has much less chance.

Sorry for the low resolution. Yes, I can post a 300 dpi 7 inch picture if you think it will help.

Thank you for your hints. I was able to duplicate your suggestions using the smudge tool on the face and then using a light gaussian blur on the background only. An improvement. Thanks.
