Gradient fade technique

I want to make an image "fade out" – at the left edge the image will be full intensity and as you move to the right the image fades out to white. I suspect one uses a gradient mask to do this but I cannot figure out how. Help!

Add a layer mask using the button at the bottom of the layers palette. Then select the layer mask and use the gradient tool to apply a gradient across it. Use the preset that has black fading to a checkerboard effect in the gradient options menu.
