And in my experience NeatImage is *much* better, and certainly much more tunable in case of pathetic image defects. Some even use it to descreen magazine scans. NoiseNinja, another decdicated noise reduction application, also seems to gets good reviews.
Both offer a version with a PS plug-in, whch works on a layer if you want. That is very effective if used with problem images, because you can use PS masks on the de-noised image layer.
