I don’t know the font, but the effect is one from a 3D program called extrusion.

The font is, quite likely, "Mata" from T-26 foundry <http://www.t26.com/typesetter.php>.

Try the program COOL 3D (ulead.com) or XARA 3D (xara.com). And extrude the font and then rotate it.

Before spending any money on a plug-in, I would go to Google and type in "Photoshop 3D text action". Try a few different ways typing it into the search engine, I know I have seen an action for this.
