have you tried a white background layer/picture layer then erase with a brush shape?

Try this: In PhotoShop, open the picture you want to add the twinkle to, then click on "Window" and scroll down to Brushes and click on it. This will open the Brushes Palette.
Click on the little boxes next to Shape Dynamics and
Scattering.(this will add a green check mark to the little boxes.) Now click on Brush Tip Shape and select a brush with a soft round shape.(I used 27).
Next, click on the words Shape Dynamics to highlight that box and open the options window. Make the following settings:

Size Jitter – 50%
Control – Pen Pressure
Minimum Diameter – 1%
Angle Jitter – 15%
Control – Off
Roundness Jitter – 0%
Control – Off

Now Click on the word "Scattering" to highlight it and bring up it’s options window. Make sure the Both Axes box is checked, then make the following settings:

Scatter – 171%
Control – Off
Count – 1
Count Jitter – 0%
Control – Off

You can now minimize the brush palette and move it out of your way.(You could just close it, but you might want to change the pattern or spacing of the twinkles.) Set the brush color to whatever you want, probably white, then use the brush to paint the twinkles where you want them. You might need to paint some, then change the brush size and paint some more.
