Yes. I just tried it in CS4

I started with a 2"x2" 72 pixels/inch jpg
It didn’t work with an 8.5" 300 pixels/inch jpg

Yes. I just tried it in CS4

ICO format only supports images up to 256×256 pixels, 8 bits/channel.

Stay tuned for a 64 bit CS4 version (though the current version should work fine in 32 bit CS4) at http://telegraphics.com.au/sw/#icoformat

For Photoshop CS4 here is the latest version of export icon plugnin
