When I resize image using ctrl+t, the image quality gets extremely low no matter how high resolution it was. Can I resize image without losing the quality?

I resize down the image and if I resize up, the image is very unclear.

In CS, when you resize down using transform, you are losing pixel data, just as when you resize an image with resampling on via Image>Size. Then when you resize up you are adding interpolated data, so you lose quality twice. With CS2, however, you can repeatedly transform a Smart Image without loss of data.

So what is the alternative for that? I have to resize to make good in document.

If it’s an entire image, and not a layer or selection, that you want to resize, use Image>Size. If you want to resize it to a smaller (or larger) number of pixels, check Resample. If you want the number of pixels to stay the same, uncheck Resample. If you resize with Resample checked, you will either lose or add pixels. Don’t do this any more times than absolutely necessary.

the real fix is to get a better image with more data to work with. might not always be possible, but it should be mentioned

Michael D Sullivan told you the answer Ishwor; look up ‘Smart Objects’ in the ‘Help centre’.
