any Adobe Acrobat expert in here?

I asked the Acrobat user group, but no one knows the answer to this. I

someone in this group could help. Here it goes:

I have Acrobat 4.0. I have a form already printed on legal-size paper. I scanned this form into a PDF file. The scanned form looks something like this:

Name: _______________

Phone: _______________

What do I need to do to be able to open this same PDF file all the time,

on the line where it says "Name:", and be able to manually type in text

and
then, click on the line where it says "Phone:", and manually type in text

that location.

Right now, the PDF file I just scanned in is read-only. I cannot type

in where the lines are.

AFAIK you can’t do that with a scanned document – a scanned document is an image and of course you cannot edit an image using vector.
I would recommend putting this on the web and using a server side programming language to accomplish this. If you’d like to stick with Adobe, her’es some tutorials:
