The letters themselves are Type; can’t remember which font. All rasterized and positioned the way they are and with Layer Styles applied.

Probably a few different layers involved.

But yes, it’s a combination of a bunch of different techniques. Not something there would be a tutorial about. I know I could do something similar, but I am also sure others could do the same job in a totally different manner. It’s more a matter of experience than one easy routine.
