lost adjustment layer palette

We recently installed a new PNY QUADRO4 750XGL video card in our dell workstation (dual zeon) replacing the wildcat card that would never perform well with 3ds MAX. while this resolved our 3ds MAX problem we now have an issue with Photoshop. when we add an adjustment layer, the dialog box can not be found anywhere on the screen. This is also true with the color picker dialog box.

PHOTOSHOP:
in Photoshop we have tried resetting the pallets locations as well as reinstalling twice and using the registry cleaner to remove any visible reg entries that may be causing the palette to be located off screen. we also changed the settings to use a single monitor. none of these procedures fixed the problem.

VIDEO CARD: currently using driver 43.03. also tried 45.23.

I don’t know what to do at this point. Photoshop is our number one application on this machine and not being able to use the adjustment layers or select colors makes it almost completely unusable.

First reset your preferences. Then read the FAQ on "Dialog Box Missing?"

If those don’t help, as a diagnostic, go to control panel and run the system applet. On the Performance Tab turn Video Hardware Acceleration to NONE. If the problem goes away, then you need to update or roll back video drivers.
