That’s just Filter > Liquify.

The flips will have to be done manually on a "mirror" layer. Not a big deal.

This is the point where someone usually chimes in with a plugin to do it in one click…

Anyway: you need to have all the elements on separate layers. Take one "sheet", cut off a corner and place on a new layer. Flip and rotate with Edit > Transform.

I just noticed there is a "warp" command in there with the transform tools, too. I haven’t used that before, but after playing around with it a little I discovered that it will do exactly what you need, and very nicely. So forget "Liquify".

The good thing about "Warp" instead of "Liquify" is that you can work on semi-transparent layers and thus easily line up with the underlying layer.

When you’re done you just have to order the layers and set the desired transparencies. Don’t flatten until final output.

Ahh…that’s a problem. I think this was new in CS2 or 3. It’s a feature of the application – not a plugin – so you can’t just download it. Unless you want to download the CS3 trial just for this.

Filter > Liquify will do the same thing, but you’ll be limited to working with one layer at a time, so it’s a bit like working blind. But it can be done if you plan things a little.

Anyway, speaking of plugins – I’m sure there is one out there.

That is definitely a "warp" effect.
Have all images of your pages on separate layers,
set the opacity to about 50%,
"ctrl T" then chose "Warp"
Use the side "guides" to give a natural flow to Ur pages if you pull on the corners it will change the proportions of Ur "page" and make it look funky
