Is there any, really? I mean without splitting hairs, here. Are there instances that call for either specific terminology?

An artist can typically illustrate or operate comfortably in freehand whereas a designer compiles and composes.

These days I don’t think so. While "designer" would suggest more composition that the creation of virgin art, there is so much blurring between roles. I’d suggest though, the a "graphic artist" is more specific, and comes under the heading of "graphic design".

On paper, a "multimedia designer"; I create compositions for display (graphic designer?), artwork and branding for print and screen (graphic artist?), video editing an animation (motion graphics?) and web sites (web design?). The titles don’t really mean much as I just see all of these as just elements of a job.
