Can anyone point me to an action or other procedure to create a raised 3D button from a photo?

A quick way is to take the Layer menu – Layer Style – Bevel and Emboss. In that dialog move the Size slider to the right until it looks the way you want. Also look at the ‘Technique’ window and click "Chisel Hard" for a more profound effect.
