HTML Forms w/ PS and ImageReady

What’s the easiest way to add HTML form (input) types to Websites created with PS and ImageReady? Can the slice tool be used to accomplish this? If not, do I have to hand-edit the resulting HTML (when saved for web) and add the input tags to the td’s?

yes, you need to hand edit the html or use an html editor.
