Layer Styles / Blending Options window hidden!

When I try to enter the layer styles window for text or a layer, nothing comes up on the screen. I can’t click on anything until I press escape (it’s like as if the window was there, and pressing escape closed it). I’ve re-installed, uninstalled and installed, disabled my 2nd monitor, rebooted, slammed head on keyboard etc.

Photoshop 7 (tried with and without 7.01 patch)
Radeon 9800 Pro using dual monitors (tried with a single monitor) Windows XP Pro w/ SP1
512MB DDRRAM
