In the Picture Package Dialog in Elements 2.0 there is an option to select page size. Three options are given none of which is the size that I use,i.e. A4 115/8" X 81/4"
Can this size be added to the options and if so how?

See "Customizing picture package layouts" in manual.

There is readme.txt file also in the
C:\Program Files\Adobe\Photoshop Elements 2\Presets\Layouts folder that is even better.
