Save for Web window displays incorrectly

When the save for web window is opened it displays like this: <http://www.cfotos.com/images/saveforweb.jpg> .

If I remove the save for web preferences from the registry the first time the window is opened it displays correctly. Succeeding openings of the save for web window display incorrectly again.

Any help with this problem would be appreciated. It makes saving transparent gif images almost impossible.

What does it look like in a 2 up view?

What kind of image is it?

Have you tried to flatten before using Save For Web and then not saving the changes? Make sure to save before flattening…

Flattening before Save for Web make no difference. This has been an intermittent problem that could be rectified by simply closing the Save For Web window and opening again. Now the only way it can be cleared up is by deleting the Save For Web perferences in the registry, but that only works for one window opening.
