First take a look at the size, in pixels, of the brush you’re using when compared to the resolution of the image you’re working on. If you are working on a 150ppi image, a brush setting of 25 pixels will look much bigger than the same 25 pixel brush on a 300ppi image. It’s a proportion thing. You can check the brush size in the Option Bar. This is one of the more common things that will throw people off.

Also, having CAPS LOCK enabled will force your cursor into "precision" mode, which is a cross-hair cursor.

Look at the preferences settings, displays and cursors. There you can choose, for the brush, outline, precision or symbol. The + is the precision setting.
