Layer mask problem

I am having a problem with layer masks in which i add one to any object and erase the contents and it leaves a faint representation of it in its place, as if it didn’t completely erase. I checked the opacity on everything, the brushes, (tried different brushes) on the layer, on the fill on EVERYTHING and to no avail, what did i do here? I have never experienced this problem in almost 10 years of working in PS. I am using cs2 9.02 btw. This is driving me nuts!!@

Check to make sure you are painting actual black.

FYI C/M/Y/K at 0/0/0/100 is not the darkest black set your colours to R/G/B/ at 0/0/0

Thanks for the response, I should have tried that however i did just hit D on my keyboard to reset the colors and that didn’t do it. I just reinstalled the program and the problem went away, who knows if it was a bug, probably not though, just my error.
