Adding detail to scanned images at chosen points or areas

Just as in Yahoo Maps/ Wikimapia/Google Earth etc. I wish to place an explanative text as a block on some chosen clickable points (or on strips of area or on larger patches ) of some old engineering TIF drawings saved in PDF. May be a place holder detail that opens by a click and vanishes by clicking on the small cross at corner of box.

How is this possible on Photoshop ver 7 converted images ? Is it also possible to embed/nest second level text / images inside? If not possible on Adobe Photoshop, possible with what other software ? (e.g., GIS ?) ?
