If you have Illustrator CS already, it will do this nicely with lots of options (a little too pricey just for this though!).

For an inexpensive solution which produces really great results, try Xara Xtreme or Xara 3D6. For 3D text work it is really very hard to beat 3D6 at anywhere within orders of magnitude of price, not to mention expended effort. For more flexibility and a greater range of output, get Xtreme. For a combo which works quite well together, get both. Check them out here
