Dissolve or Disintegrate Effect

Does anybody know how to create a dissolve or disintegrate effect where a graphic breaks up into smaller and smaller squares? This can be in either Photoshop or Illustrator.

I’ve seen this effect in a lot of logos but I can’t seem to find one to illustrate what I’m talking about. Sorry.

Any help would be appreciated,

Maybe something like the Alphastrip plugin?
